Investigation and management of long-standing chronic constipation in childhood.

The anorectal physiology of 106 children with long-standing chronic constipation, who had failed to response to a trial of medical treatment, was assessed. 10 (9%) were shown to have ultrashort-segment Hischsprung's disease, later confirmed on histology, The remainder showed evidence of hypertrophy of the internal sphicter on anorectal manometry and had a vigorous anal dilatation (to accept 4 fingers) under general anesthesia. After this, 38% were able to be weaned off all medication and most of the remainder improved. Further anal dilatation and internal sphincterotomy allowed a further 10 children to stop laxative, bringing the total to 48%.

The abuse of laxatives and a prolonged treatment with diuretics has to be brought into consideration as the most common cause for renal or intestinal loss of potassium. Besides characteristical alterations at the E.C.G. and besides intestinal disturbances there do occur again and again acute, life-threatening aspects of cases connected with tetraplegias and a respiratory failure. By means of 3 cases from our hospital and 27 casuistics in literature symptomatology and dynamic in the development of hypokaliemia is discussed. The mean of potassium in the serum of the 16 patients, those having quadriplegias, ran up to 1,7 mval/l (range 1,4-2,5). Paralysis develops peracutely in 4 of the cases within hours and in 12 of the cases within days. In the anamnesis symptoms of adynamia could be traced with nearly every patient. A functional psychosis (reversible physically founded psychosis) couldn't be detected in any of the cases. With the help of a administration of potassium one could achieve a total retrogression of the symptoms. Besides this a normalisation of the acide-base equilibrium is required because of a metabolic alkalosis, detectable in most of the cases.

A survey of colonic pathology with a double contrast method and analysis of the indication for a barium enema.

A prospective study of colonic pathology with double contrast method was made of 146 patients sent to the X-ray department of the Black Lion Hospital, Addis Ababa. The barium enemata were done for complaints like constipation, diarrhoea, rectal bleeding and palpable abdominal mass often without any relevant prior physical clinical examinations. Of those examined only 26 (17.8%) patients were found to have radiologically colonic pathology and the rest, 120 (82.2%) patients were reported normal. The policy of prior clinical examination before any barium enema such as rectal digital examination, sigmoidoscopy and stool occult blood studies would reduce the normal barium enema. The survey compared bowel preparation with adequate fluid intake and laxative without cleansing enema serves as a good method of bowel preparation for ambulatory patients.
