The results of 1300 radiographic examinations of the colon with the double-contrast method were reported. In preparation for the examination, only a laxative (cascara salax) was administered and the patient not permitted to eat. The importance of horizontal films was mentioned in particular. A malignant tumor in the colon was discovered in 137 cases. A malignant degeneration was established via histologic examination in 23 of the 152 patients with polypoid lesions. Juxtaposition of a malignant tumor and a polypoid lesion or a second carcinoma was discovered via radiological examination in 28 patients.

Managing children with constipation: a community perspective.

Two clinical nurse specialists explain the nature of constipation in children and how the primary health care team can achieve successful treatment and management strategies for this condition. Constipation in children is a common and often complex problem. It may begin at four months of age in relation to weaning, or at around two years of age in relation to potty training. Constipation can be a distressing problem for the child and the family. Treatment failure rates are high, frequently reflecting poor understanding of the pathophysiology of constipation and inappropriate management. Symptoms include infrequent defaecation, pain and distress and refusal to defaecate. Causes include a poor intake of dietary fibre and fluid, emotional disturbances, possibly intercurrent infection and change in routine. Management of children with constipation includes an increase in dietary fibre and fluids, behaviour modification and laxative medication. For treatment to be effective there should be regular follow-up. Management of this chronic problem by nurses is viewed as effective and acceptable to parents.

Colostomy closure after Hartmann's procedure with fast-track rehabilitation.

PURPOSE: The aim of this study was to assess the effect of postoperative multimodal rehabilitation after colostomy closure after Hartmann's procedure. METHODS: Twenty-seven consecutive patients scheduled for colostomy closure after Hartmann's procedure received continuous postoperative epidural analgesia, laxative, and enforced oral liquids, protein drinks, and mobilization. RESULTS: Defecation occurred at a median 2 days postoperatively. Mean and median postoperative stay was 3 days, with two readmissions (1 anastomotic dehiscence and 1 social) increasing mean total 1-month hospital stay to 4.3 (median, 3) days. No other complications occurred except three superficial wound infections. CONCLUSION: Postoperative hospital stay and morbidity may be reduced with multimodal rehabilitation after open colostomy closure after Hartmann's resection.
