Constipation in the elderly.

Constipation is a frequent complaint among the elderly. Misconceptions about normal function and insistence on a daily bowel movement may be factors which have led to the overuse of laxatives. Evidence of gastrointestinal blood loss, a recent change in bowel elimination pattern or complications of constipation are indications for evaluation by complete laboratory, endoscopic and radiographic procedures. Natural fiber, careful attention to diet and fluid intake, regard for toilet needs and encouragement of activity are recommended as effective means for correcting constipation.

Hormonal and biochemical abnormalities in women suffering from eating disorders.

This study reports the results of a biochemical investigation of 80 eating disorder patients and results of an endocrinological investigation of 20 subjects. Of the 80 subjects studied, 22 suffered from anorexia nervosa and 51 were diagnosed as having bulimia. These patient's results were compared to those of 30 control subjects. The eating disorder patients had significantly higher levels of total CO2 calcium, alanine aminotransferase and cholesterol, and significantly lower levels of potassium, chloride and phosphate in the plasma. Hypokalaemia was strongly associated with self-induced vomiting and laxative abuse. Hypercholesterolaemia occurred most commonly in anorexia nervosa patients. Preliminary endocrinological results suggest decreased gonadotrophin levels are associated with binge eating and self-induced vomiting and laxative abuse, as well as with low weight. We feel eating disorder patients should be interviewed and examined by a physician with an interest in this area. Appropriate investigations should be ordered. The physician should also undertake counseling about the short- and long-term sequelae of disordered eating.

Binge-eating and self-induced vomiting in the community. A preliminary study.

This study examined by questionnaire the prevalence of binge-eating and self-induced vomiting among a sample of 369 consecutive attenders at a family planning clinic. 20.9 per cent reported current episodes of uncontrollable and excessive eating ('binges'), 2.9 per cent currently induced vomiting as a means of weight control; and 4.9 per cent reported using laxatives. Binge-eating and self-induced vomiting were strongly associated with disturbed attitudes to food, eating, body weight and shape; and with psychiatric disturbance. Menstrual dysfunction was not associated with either body weight or binge-eating; it was however associated with psychiatric state. Applying conservative rules, 1.9 per cent appeared to fulfil diagnostic criteria for the recently described syndrome bulimia nervosa.
