[Bowel control after surgery for Hirschsprung's disease]

Bowel control was studied in 66 children operated by Ikeda's Z-shaped anastomosis for Hirschsprung's disease. At the age of 2 or 3 years, mild constipation was recognized in 16% of the children, incontinence in 12% and soiling in 28%. However, these disorders reduced as the patients grew up and finally over 7 years, constipation was seen in 8% of the patients, incontinence in 8% and soiling in 19%. According to barium enema, megarectum was seen in all constipated children but one, though slight megarectum was sometimes seen even in normal or incontinent children. Frequently, anal canal was wide and leakage of barium through the anus was seen in incontinent or soiling children. Anal canal pressure was high in constipated group, low in soiling group and the lowest in incontinent group. The appearance rate of rhythmical wave of anal canal and recto-anal reflex was the same among all groups. Our treatment of the constipation and incontinence is daily bowel movement using laxative, suppository or enema. For the incontinent patients without rectal sensation, biofeedback method or electrical stimulation is applied.

Short-chain fatty acids and bowel functions in man.

The fecal excretion of short-chain fatty acids (SCFAs) (measured by gas chromatography), transit time (assayed by radiopaque plastic markers), fecal weight, intake of dietary fiber, and their relationship have been studied in 10 healthy subjects during 1 week. The subjects had a fiber intake (mean +/- SD) of 26.3 +/- 7.9 g/day, a fecal weight of 160 +/- 49 g/day, and an average transit time of 41 +/- 9 h and excreted 11.1 +/- 5.5 mmol of SCFAs/day, with a concentration of 66.7 +/- 28.0 mmol/kg feces. The mean amount of SCFAs, but not their concentration, correlated to mean fecal weight (p less than 0.01) and mean transit time (p less than 0.01). The mean fecal weight correlated to mean transit time (p less than 0.05). The mean intake of dietary fiber did not correlate to any of the other variables. The results of this study indicate that other substrates are more important than fiber for the bacterial formation of SCFAs in the colon. The SCFAs formed most likely do not exert any direct laxative effect.

Exogenous factors influencing the human erythrocyte sodium-lithium countertransport system.

It has recently been found that the Na+-Li+ countertransport across the human erythrocyte membrane is increased in patients with essential hypertension. We investigated the influence of hypokalaemia, oral contraceptives, diabetes mellitus and essential hypertension on the activity of this transport system. Normal values for the maximal Na+-Li+ transport rate were 0.25 +/- 0.08 mmol l-1 h-1 (males, n = 18) and 0.23 +/- 0.06 (females, n = 14). We found elevated values in women taking oral contraceptives (0.34 +/- 0.07, n = 10, P less than 0.001), in patients with chronic hypokalaemia due to diuretic or laxative abuse (0.41 +/- 0.16, n = 13, P less than 0.005) and in those with essential hypertension (0.32 +/- 0.08, n = 24, P less than 0.001) (all data mean +/- SD). Thus our results with hypertensive patients support the findings of other investigators. However, oral contraceptives and drug-induced hypokalaemia greatly modify this system, indicating a regulation of the Na+-Li+ countertransport by hormones. Thus the transport rate does not seem to be an appropriate test for the diagnosis of essential hypertension.
