At present, reduction of the consumption of fat is the main issue in nutritional education. However, this can lead to an increased consumption of mono- and disaccharides and, as a result, to increased dental caries. Individual variations in the susceptibility of caries make it necessary to differentiate the advice for the consumption of snacks between meals. Generally, consumption of complex carbohydrates, such as bread or bread products, can be advised as an alternative for fat and sugar containing snacks. Sugar-free products can also be advised, but not unlimited because of the laxative effect of sorbitol. To prevent tooth erosion the frequent consumption of fruits and fruit juices should be discouraged.

Faecal impaction: older people's experiences and nursing practice.

A number of authors have identified the paucity of evidence available to inform community nursing practice in relation to faecal impaction. Little is known of its incidence or its impact. In an attempt to address this, this article reports part of an extensive descriptive survey in Australia that sought to explore the experience of, and response to, constipation of older people living in the community. The focus here is the participants' experience of extreme constipation in the form of faecal impaction and obstipation (obstinate constipation). Faecal impaction was found to cause great discomfort and distress, often leading to obstipation. The imperative for self-management of constipation was strong, and laxative use frequent. However, manual evacuation was often necessary, either by self or others. This article discusses the results in relation to a recent emergence of concern in the US and UK about how faecal impaction and obstipation should be treated by community nurses.

Food preferences and aversions and dietary pattern in anorexia nervosa patients.

Food intake, meal frequency and occurrence of vomiting and purging were assessed in 20 female patients with anorexia nervosa (AN) by two dietitians by means of a dietary history over the past month. Ten lean healthy women and 10 of normal weight served as two groups of controls. Two self-administered questionnaires, consisting of 51 questions each were developed and applied: one to get an insight into food preference and food aversion, another to evaluate the dietary pattern. A significant difference in energy intake was found between the patient group and both control groups, but not between the two control groups. Self-induced vomiting and weekly laxative use was only mentioned by the patients with AN. Between the patients and the controls significant differences were found in preference and aversion for several food products such as sugar, potatoes, meat, dates and bananas as well as in eating habits. Stepwise discriminant analysis of the data of the two questionnaires resulted in eight food items and three dietary pattern statements which specify group differences. It is concluded that the questionnaires used reveal important differences between anorexia patients and control subjects in attitudes towards foodstuffs, and can be a useful tool for early diagnosis when validated in larger samples of patients and controls.
