[Comparative evaluation of a new colon preparation method prior to a double-contrast barium enema: toward discontinuation of the cleansing enema]

Evaluation of the efficacy and adequacy for double contrast barium enema of a recently described oral lavage solution associated with bisacodyl (Prepacol) and its comparison with a traditional method of colon cleansing (low-residue diet, cathartics and water enema) are reported. Two similar groups of 100 outpatients were compared. The new cleansing method presents several interesting advantages: time saving for the nurse of the radiology department and the patient himself (cleansing enema is no longer required), better patient acceptance, equal evacuation of fecal residue but significantly better evacuation of residual liquid resulting in better mucosal coating.

Sodium phosphate preparation for colonoscopy: onset and duration of bowel activity.

BACKGROUND: Two doses of sodium phosphate are recommended for colonic cleansing before colonoscopy. Patients need information about the onset and duration of activity of the preparation. This study was designed to develop practical prescribing information to patients to reduce anxiety concerning bowel accidents. METHODS: Two hundred patients were requested to take a liquid diet the day before colonoscopy, to take 45 mL sodium phosphate with water or juice the evening before and the morning of the procedure, and to complete a questionnaire. RESULTS: Bowel activity began 1.7 hours (range 0.5 to 13) after the first dose and continued for 4.6 (1 to 14) hours. For the second dose the time to onset was 0.7 hours (0.25 to 4), and bowel activity continued for 2.9 hours (0.5 to 6.5). Bowel activity was completed 4 hours after ingestion of the second dose in 83% of patients and within 5 hours in 87%. Three of 200 patients reported the need for bowel evacuation during transit. CONCLUSIONS: The activity from the second dose of sodium phosphate starts and ends sooner than that from the first dose. When advising patients on when to take the second dose of sodium phosphate, 4 hours for most and 5 hours for those with long commutes, should provide adequate time for almost all patients to avoid bowel activity during travel.

Evaluation of rapid colon preparation with Golytely.

The traditional cleansing method of the large bowel usually involves dietary restrictions, purgatives and enema. They are time consuming and uncomfortable, and may result in dehydration. Though Golytely, an oral lavage solution, has been developed since 1980 as an alternative for cleansing the colon and much literature has favored it, it has not been widely used in Taiwan. In this study, we used Golytely without enema for rapid colon cleansing in a consecutive series of 48 patients. The efficacy, change of vital signs, body weights, electrolytes, and urine specific gravity were evaluated. Patient's acceptance of this method was compared with that of 32 patients who received castor oil as cleansing method. The average amount of Golytely used in this preparation was 3.16 liters, and the average time was 8 hours and 43 minutes. About 87% of patients receiving Golytely achieved a good to excellent preparation. There were no significant changes in vital signs, body weights, electrolytes and urine specific gravity. The patient's acceptance of Golytely was better than that of castor oil.
