[Prospective study of preparation for colon double-contrast studies with enemas compared to oral mannitol/Na-picosulfate administration with the same diet]

In preparation for the colon double contrast method two groups of patients, both with identical dietary regimen, were compared. In one case an oral mannitol dose (20%) plus sodium picosulphate was given, in the other enema plus bisacodyl was applied. In a total of 132 patients, the group which was treated with mannitol showed significantly better cleansing results and a better coating with the contrast medium. Considerable side effects did not occur. Further advantages: reduced number of personnel required, less trouble for the patients, out-patient investigation possible.

A simple regimen for control of postoperative sepsis in colorectal surgery.

A preoperative schedule based on three principles: rigorous mechanical cleansing of the intestine, nutrition for four days with a formula diet (Biosorbin MCT), and short-time systemic prophylaxis with doxycycline has been tested in a prospective study of 252 patients who underwent elective colorectal surgery. There were 11 wound infections, most of them of minor importance (4.4 per cent). Intra-abdominal abscesses occurred in two patients, both successfully reoperated, giving an overall incidence of septic complications of 5.1 per cent. The schedule was easy for the staff to administer, well tolerated by the patients, an inexpensive. The complication rate is probably as low as could be anticipated with this type of surgery.

Low residual diet and hydration improving double contrast examination of the colon.

Light food diet and low residual diet with hydration, both combined with Proctosal and Bisacodyl cleansing, were compared in 268 patients in the preparation of the colon for double contrast examination. Low residual diet with hydration resulted in significantly less residual fecal material, no flocculation of the barium coating and significantly denser mucosal coating. The examination had to be repeated more often (8.6%) after light food diet than after low residual diet with hydration (1.7%), but there was no statistically significant difference in the diagnostic accuracy. The hydration is important in avoiding patient discomforts and flocculation of the barium coating. Despite the use of laxatives, a strict diet restriction is needed to obtain consistently clean colon.
