  • Mail Order: Print and fill out the Order Form, and mail it to us along with payment in cashier's check or money order drawm in US dollar (or personal check from US banks) to DreamPharm, P.O. Box 9470, Glendale, CA 91226-9470, USA. Include shipping cost for Overseas shipping (shipping to countries outside of US).
