Comparison of multivariate calibration methods to determine simultaneously mebendazole-cambendazole and mebendazole-thiabendazole in pharmaceutical preparations by UV-visible spectrophotometry.

Three multivariate calibration methods, Principal Component Regression (PCR), the K-matrix method and Q-mode factor analysis followed by varimax and Imbrie's oblique rotations were applied to the simultaneous spectrophotometric determinations of mebendazole (MBZ)-cambendazole (CBZ) and thiabendazole (TBZ)-mebendazole in commercial samples of Exelmin and Helmiben. The calibration set concentrations were selected to contain a +/-10% variation in the quantity of active ingredients as declared by the manufacturer. The Q-mode factor analysis provides superior results for the two pharmaceutical formulations. The K-matrix method proved to be totally inadequate for these determinations. Almost all Q-mode results have relative errors much smaller than 5% of the active ingredient contents. This investigation shows that PCR and Q-mode factor analysis can be used to determine MBZ-CBZ and TBZ-MBZ in commercial drugs.

Abdominal angiostrongyliasis is a zoonotic infection produced by a metastrongylid intra-arterial nematode, Angiostrongylus costaricensis. Human accidental infection may result in abdominal lesions and treatment with anti-helminthics is contra-indicated because of potential higher morbidity with excitement or death of worms inside vessels. To evaluate the effect of mebendazole on localization of the worms, male Swiss mice, 5 week-old, were infected with 10 third stage larvae per animal. Twelve infected mice were treated with oral mebendazol, at 5 mg/kg/day, for 5 consecutive days, begining 22 days after inoculation. As control groups, 12 infected but non-treated mice and other 12 non-infected and non-treated mice were studied. The findings at necropsy were, respectively for the treated (T) and control (C) groups: 92% and 80% of the worms were inside the cecal mesenteric arterial branch; 8% and 10% were located inside the aorta. Only in the group C some worms (10%) were found inside the portal vein or splenic artery. These data indicate that treatment with mebendazole does not lead to distal or ectopic migration of A. costaricensis worms.

Diagnosis and treatment of hepatic echinococcosis: an overview.

BACKGROUND: Surgery has long been considered the first-choice treatment in patients with echinococcosis of the liver. The poorly predictable outcome of older studies using mebendazole or albendazole confirmed this belief. Since the introduction of a percutaneous technique (PAIR; puncture, aspiration, injection, reaspiration) treatment policy is changing. The actual question is which treatment is preferred in which patients. METHODS: Review of recent literature. RESULTS: Laparoscopic treatment of anteriorly located hepatic cysts is a new surgical technique with high success rates (77%-100%) and low complication (0%-17%) and recurrence rates (0%-9%). Albendazole is superior to mebendazole treatment. Degenerative changes were found in 82% of patients treated with albendazole and in 56% of those treated with mebendazole. The main problem is the high relapse rate: 25% mostly within 2 years. PAIR proved to be superior to albendazole treatment (88% versus 18%) and equally effective as surgery (86% versus 76%). A combined injection of alcohol with polidocanol is a simple alternative to PAIR. Percutaneous evacuation of cyst content (PEVAC) made percutaneous treatment accessible to patients with complicated cysts. CONCLUSIONS: Albendazole is the first-choice treatment in patients with univesicular cysts. PAIR or combined injection of alcohol with polidocanol is indicated when pain is intractable or albendazole fails. In patients with multivesicular cysts, PEVAC is a better choice. Surgery is the first-choice treatment only when the expertise of percutaneous treatment is not available or when percutaneous treatment fails.

The effect of intraoperative mebendazole-albendazole applications on the hepatobiliary system.

Caustic sclerosing cholangitis occurs with the use of scolicidal solutions in liver hydatid disease draining into the biliary system. In this study, we investigated the effectiveness of benzimidazole solutions, their in vitro scolicidal effects and the histopathological changes in the hepatobiliary system due to their intraoperative use. It was found that 5 mg% mebendazole and 1 mg% albendazole have strong scolicidal effects. In an in vivo study, under general anesthesia, 5% mebendazole in group I, 1% albendazole in group II and normal saline in group III were injected into the biliary system of rabbits. Liver biochemical tests showed no significant changes. More elaborate ductal mucosal proliferation, ductal dilatation and periductal fibrosis were found in group I compared with group II in biopsies taken on the 60th day. The biopsies of group III were normal.

Ascariasis pneumonitis: a potentially fatal complication in smoke inhalation injury.

Ascaris pneumonitis in areas of endemic infestation is considered a benign condition. Smoke inhalation with any burn injury can be potentially fatal. A heavy infestation of Ascaris could further exacerbate the smoke-induced lung injury. After ingested eggs hatch in the small intestine, the larvae penetrate the mucosa and invade the blood stream and are then carried to the lungs. The larvae break out into the aveolar spaces as they are too large to cross the capillary bed and are carried up the bronchial tree and eventually swallowed. This study describes three cases of Ascaris infection in thermally injured children. While the burns were < 30 per cent total body surface area, two patients who were injured in the same fire had a further complication of smoke inhalation which necessitated sophisticated therapy in order to promote survival. All patients were treated initially with Vermox. The one patient without smoke inhalation did not develop ascariasis pneumonitis even with positive stool samples and was discharged with no complications, whereas the two with smoke inhalation developed severe pneumonitis. One patient was placed on ECMO and did not receive a full course of the Vermox treatment. This patient died after several weeks of ECMO treatment. The third patient received a full course of Vermox, slowly recovered, and went home. Supportive therapy only is recommended during the lung migration phase of the Ascaris lifecycle. We feel that continuation of chemotherapy (Vermox) would have been beneficial in the fatal case based on the survival of the second patient.(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluation of mebendazole activity on experimental murine toxocariasis by immune complexes determination.

The mebendazole action on experimental murine toxocariasis, using different formulations and vehicles, was studied by means of the detection of antibodies and immune complexes by ELISA. After inoculation with 1000 embryonated Toxocara canis eggs, BALB/c mice were submitted to the anthelmintic treatment as follows: group 1 (control without treatment); group 2 (mebendazole (MBZ), Lomper, Steve Laboratories, Barcelona, Spain, suspended in 1% sodium carboxymethylcellulose (CMC) at a dose of 100 mg/kg/day); group 3 (MBZ, pure compound suspended in 1% CMC at a dose of 100 mg/kg/day); group 4 (MBZ, pure compound suspended in water at a dose of 100 mg/kg/day); group 5 (MBZ, pure compound, formulated to a solid dispersion at 10% in polyethylene glycol 6000 (PEG) then dissolved in water at a dose of 100 mg/kg/day); group 6 (MBZ, pure compound, formulated to a solid dispersion at 10% in PEG then dissolved in water at a dose of 50 mg/kg/day); group 7 (MBZ, pure compound, formulated to a solid dispersion at 10% in PEG then dissolved in water at a dose of 25 mg/kg/day). The treatments were administered on days 5-7 post-inoculation (p.i.) inclusive. The dynamics of the production of the specific antibodies for both excretory-secretory (ES) antigen or crude extract showed a similar profile as compared to the control group. In groups 2 and 6, from the beginning of the treatment, values of immune complexes fell rapidly and were undetectable for the remainder of the experiment. Reductions of immune complex levels by the 4th-6th, 2nd-3rd and 2nd-5th weeks p.i. were observed from groups 3, 5 and 7, respectively. In the other groups, similar profiles as compared to the control group were observed in the dynamics of the specific immune complexes. The evaluation of chemotherapy by immunological methods is a valid technique for the efficiency of the treatment without the disadvantages of larval recovery from several digested tissues. Mebendazole, pure compound, formulated to a solid dispersion in PEG, then dissolved in water reduced immune complexes from the beginning of the treatment. The larval immobilization produced by MBZ should entail a reduction in their metabolic activity with a reduction in the production of excretory-secretory substances which are responsible for the formation of immune complexes. The rapid clearance of specific immune complexes together with a total larvae reduction would explain the decrease in specific immune complexes, which detection is a valid technique for monitoring the efficiency of treatment.
